Most commentators will say this verse is hyperbole. We aren't actually instructed to pluck out our eyes or cut off our hands, rather we must have a spiritual willingness to rid oneself of sin. I agree that our Lord was using extreme language but I don't necessarily agree that it is only has a spiritual application.
The bible is full of verses about putting God above all else. We must be willing to give up everything: wealth, safety, family, our lives, everything for the sake of Christ. Indeed an eye or a hand would be a small sacrifice compared to the sacrifices Christians have made and are making today.

We live in the age of technology. Many people consider their smartphones, tablets and computers to be extensions of themselves. I can't help wonder what would happen if an EMP or solar flare disabled our internet, cellphones and power grid. I imagine quite a few people would suddenly lose their reason for living. What I am going to suggest will be unimaginable for some.
Get rid of your smartphone.
And the internet, netflix, cable tv, magazine subscriptions, etc. Whatever it is that is creating constant temptation needs to be cut out of your life for at least an extended period of time.
If you are currently addicted to porn you must cut out the most frequent sources and rid yourself of the access and temptation. This is what the Lord was saying about the eyes. If you aren't willing to sacrifice something you believe is essential (such as an eyeball or hand) to walk with God and rid yourself of sin than you aren't worthy of the kingdom.
This is also the meaning behind the parables of the rich young ruler (Matthew 19:16-22) and hating your family (Luke 14:26).
